CDC’s Injury Center created the STEADI Tool Kit for health care providers who see older adults in their practice who are at risk of falling or who may have fallen in the past. The STEADI Tool Kit gives health care providers the information and tools they need to assess and address their older patients’ fall risk. The CDPHE Older Adult Falls Prevention Project is using the STEADI Toolkit to engage health care providers in the target area of Adams and Arapahoe Counties.
The downloadable STEADI materials include handouts for patients as well as tools any health care provider can use such as Talking about Fall Prevention with Your Patients, patient case studies, and how to use validated tests to asses your patients' falls risk factors.
